Register as a patient
If you wish to register with Edenbridge Medical Practice and live within our catchment area,you can either register online or you can come to the reception desk and complete the necessary forms. Please bring a photo ID and proof of current address with you.

• Completing the registration forms online does not automatically mean that your registration has been accepted.
- You must make sure that you complete your previous address in the UK on the registration form to avoid any delay.
• It can take up to 5 working days to process requests to register, it will not be actioned immediately due to the high number of new patient registrations being received.
New patients will be registered with a named GP and will always see or speak to that GP unless he or she is away from the practice.
The Doctors are committed to do everything possible to protect staff, patients and visitors from unacceptable behaviour and support a zero tolerance policy to any incident that causes hurt, harm damage or stress. This policy applies throughout the premises of Edenbridge Memorial Health Centre, including the car park and surrounding grounds. It also applies to any GP or employee of the Practice, whilst on duties away from the surgery if their duties are related to the business of the Practice.
Temporary Registration
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. You will need to attend the Practice to fill out the necessary forms. This cannot be done online. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.
If you wish to use our Minor injury service and you are not registered as a patient here, you will need to complete a temporary registration form, which is available at reception.
Your Named GP
The Surgery provides all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them.
